Care Coordinator / Support Worker

PERMANENT COMMUNITY CARE COORDINATOR REQUIRED TO WORK FOR A PRIVATE HOME CARE CLIENT IN PALMERS GREEN, ENFIELD.

Applicant must drive, have access to a car and live no more than 30 minutes from the service.

Annual Salary: £18,000- £19,000 pa+ extra pay for weekend out of hours

, Monday to Friday: 09:00-17:00

, The manager is looking for the position to be filled ASAP.

Daily Duties:
The client cares for and supports adults with mental health conditions, along with supporting those with Alzheimer's and other forms of dementia and learning disabilities.

As a care coordinator you will be out and about within the community visiting the Elderly (65+ years of age) within their own homes to support them to live ad independently as possible.



Responsibilities will include:
, To carry out monitoring visits to clients as requested by the Care Manager
, Provide support and advice to service user and inform carers.
, Give support to members of the service user's family both psychologically and in gaining knowledge and information
, Participate in the promotion of healthy living and the prevention of ill health and disability
, Participate in the teaching and assessing of care staff within the care package
, Participate in teaching, induction and orientation programmes for new staff
, To report sickness, absence or unprofessional behaviour to a senior member of staff
, Interviewing potential Care Workers by following interview guidelines and checklist
, Taking new Service Users referrals from social workers, private Service Users, and contracting organizations
, Assigning Care Workers to Service Users, adhering to Core Outreach policies and principles
, Responsible for all data input relating to specified area as changes occur.


, Ensuring that holiday/sickness and emergency calls are assigned.


, Updating records of Service Users and Care Workers on an on-going basis on computer programs

Person specification:
The applicant must have a clear DBS and have recent and relevant experience and an NVQ in Health and Social Care.
