Service Care Solutions have a new and exciting opportunity working within the East of Lincolnshire area joining their Community Care team!

Within this role you will be required to travel throughout the Lincolnshire area to provide support to hospitals,residential homes and clients homes to assess each person on a individual basis and help to provide a care plan that will work for them.

You will need to be a driver for this role as you will be out and about in the community assisting with things such as care plan assessments/production, supporting hospital discharge and providing care reports for people in their own homes from scratch!

Each case will be different so they are looking for someone who can assess each client on a case by case basis and ensure that all angles have been covered in terms of thie health, their saftley and their needs.

This role is a full time role working Monday to Friday 9am-5pm currently paying £12.87 and 45p per mile

This role is a very busy role where you will need to be on your toes and happy to be a busy bee!

Requirements for the role:

Enhanced DBS

Must be a driver

Previous experience with producing care plans from beginning to end - ideally for the local authorities

Happy to travel (miles will be paid)
