Social work assistant
I am currently recruiting for a social work assistant role based in Hackney.
This role is intially for 3 months and the applying candidates should have an enhanced DBS.
PURPOSE OF JOB
To provide support, information and advice and technical assistance to individuals and to develop, cost, negotiate and mediate their individual support plans as required.
To enable individuals to take control of their support plans and resources.
To assess people who are eligible for social care support to develop their own solutions and enhance their quality of life through promoting choice.
To work jointly and also alone to develop support plans with people and implement these to enable people to be healthy, safe and well.
, Using person centred approaches, enable people to develop outcome focused support plans that meet their identified needs , To support individuals to identify potential risks and find solutions.
, To assist people to find creative solutions to meet their identified needs
, To present information about the support plan, and where relevant with the service users, to other professionals and relevant panels.
, To help people to put their support plans in place in order to meet their needs
, To work with all relevant people and organisations in support of a person to ensure their needs are met effectively, responding to timescales as appropriate
, To ensure, where necessary that all relevant information is communicated to key stakeholders/providers to ensure a person's needs are effectively met.
, To identify safeguarding risks and instigate safeguarding procedures as required.
, To act as a source of information and advice to team members in relation to support planning
, To support informal/family carers and where appropriate assess and implement a carers service in line with eligibility criteria
, To ensure a high level of data quality for all information recorded, ensuring that all information is timely, accurate and complete
, To provide advocacy for individuals in relation to the self directed support process and the implementation of their plans
, To use good communication skills to work with the person, their family to enable them to develop their support plan
, To ensure that knowledge is updated in relation to national and local drivers in social care and attend training where necessary.
, To participate in personal supervision as Council organisation procedures
, To take responsibility for ensuring that the Promoting Independence Commitment Statement is signed and is personally committed to in order that it is identified as part of the performance framework for this post.
