Social Support Manager


A housing association based in Sevenoaks is recruiting for an experienced Social Support Case Manager to deliver high quality support to individuals in their own homes, working flexibly with clients supporting them to manage correspondence, shopping, maintain tenancies, appointments and relationships with others.



The Role


The focus of this role is supporting residents of the housing association in maintaining their independence.




Support includes helping clients to deal with correspondence, ensuring important matters are dealt with on time, managing finances including taking clients to the bank and ensuring the full range of benefits are being claimed, shopping including writing a list and encouraging healthy eating, taking to appointments (no personal care)


  • One of our most important roles is to be a listener, allowing clients to talk about things without interruption, allowing them the time (maybe over more than 1 visit) to express their needs, their worries, their aspirations.


  • Support hours for individuals range from 1 to 6 per week, some clients will have set days/times whilst others will be more flexible

  • You will have a set client group but will be expected to cover colleagues during periods of leave or sickness

  • Recording visit notes, risk assessments and support planning




This will include the following tasks and responsibilities:


  • Deliver an excellent support service to a range of vulnerable clients to achieve agreed outcomes within agreed timescales.

  • Identify and collaborate with the most appropriate partners to achieve positive outcomes for clients.

  • Ensure all relevant paperwork regarding clients and the service delivery are maintained to an auditable standard.





The Candidate


To be considered for this Social Support Case Manager role it is essential that you have previous housing related support experience and be able to build a rapport with vulnerable clients and handle challenging behaviour.


The role works with residents in their own homes across the Sevenoaks district, Tunbridge Wells, Tonbridge, Dartford and Maidstone you will require access to your own vehicle for work purposes.


As this role includes work with vulnerable individuals, you will require an Enhanced DBS on the update service.



The Contract


This is a part time Social Support Case Manager role, working 9-5 Monday to Friday on an initial 3 month contract.
