Lead Social Worker
An amazing new job opportunity has arisen for a committed Lead Social Worker to work in an exceptional hospital based in the Edmonton, London area.
You will be working for one of UK's leading health care providers
This hospital provides a full care pathway including Medium Secure and Low Secure units for males, along with a mixed gender acute wing.
The full multidisciplinary team works with those who have a diagnosis of mental illness and associated challenging behaviours
As the Lead Social Worker your key responsibilities include:
· Identify any social issues which a patient may be experiencing and ensure these are addressed within risk assessments and care plans and make sure the patient is supported
· Create links between the care team on the ward and the patient’s community team, ensuring that early discharge planning is arranged and community teams are fully up to date and aware of the care pathway of the patients
· Develop links between different agencies within a patient’s local community, for example Care Coordinators, MAPPA, families/carers and future possible placements
· Act as the Hospital’s Designated Safeguarding Officer, maintaining links with Enfield Council, escalating safeguarding concerns and monitoring any ongoing safeguarding concerns
· Take a proactive approach in supporting the wards within the hospital with monitoring any safeguarding concerns amongst the patient group on the ward, ensuring the safety of patients at all times
· Complete investigations into incidents in the hospital and feedback this to the Senior Management Team
The successful Lead Social Worker will receive an excellent salary around £42,000 per annum DOE.
This exciting position is a Full Time role working 37.5 hours a week.
