Band 6 Occupational Therapist

Service Care Health are looking for a Band 6 Occupational Therapist to work in the community around Longridge Community Hospital.

You can expect to be earning £24 per hour.

The role is Monday - Friday 9am-6pm (hours flexible), although the trust will also consider part time hours.

You will be working as part of the CATCH Team (Central Allocation Team for Health and Care) and the Home First Multidisciplinary integrated team which includes both health and social care colleagues working together to ensure appropriate services are commissioned to suit the patient's needs.

More about the role:
- You will improve experience for patients and their families in relation to the discharge process
- You will promote continuity of care from the acute into the community setting
- You will enable patients to recover within their home environment after assessment of their needs within their usual place of residence
- The contract is initially for 3 months, with a likelihood of it being extended

Requirements
- HCPC registration
- Relevant qualification in Occupational Therapist
- Access to a vehicle for work purposes
- Ideally previous experience as an OT in the community
