Job Title: Serious Incident Investigator
Duration: 6 months extendable to 12
Hours: 37.5 Hours PW
Location: East Sussex
Brief description of role:
, Developing and delivering appropriate training programmes in aspects of patient safety for all grades of staff (clinical and non-clinical) involved in care delivery
, Conducting Serious Incident investigations using a root cause analysis (RCA) methodology.
Ensuring that an accurate report is produced along with a robust SMART action plan, within the specified timescales and updating the Datix system accordingly.
This will involve contact with affected staff, patients, relatives and bereaved individuals, requiring tact and empathy
, Contributing to the development of recommendations and actions following serious incident investigations
, Developing analysis and reports, and presenting complex, sensitive and contentious information to groups, committees or other forums as required, both internal and external to the Trust
, Drafting complex reports outlining the findings of investigations and compile robust action plans in conjunction with the appropriate
, Supporting staff in the preparation of statements for serious incidents (SI) investigations and inquests as necessary
, Identifing patient safety topics for audit and leading the development of the audit tools and audit process including completion of the change cycle.
, Leading, co-ordinating and participating in promotional events to reduce and prevent harm in the delivery of care
, Developing IT skills in accordance with ESHT Information Governance guidelines to promote the quality development of the service e.g.
Word processing, ExCEL spread sheets, database inputting and Power Point usage.
, Educated to Masters level or significant demonstrable experience of working at a similar level in specialist area.
, At least two years' experience working in patient safety/risk management
, Working within a multi-disciplinary settling
, Demonstrable experience of leading projects/investigations in a complex and challenging environment
, Ability to appraise research findings and apply to practice
, Working knowledge of DATIX system
, Experience of project work
, Experience of audit within healthcare settling
, Clinical qualification and current registration with the relevant professional governing body
