Nurse Team Leader

We are looking for Band 6 Community Nurse Team Leaders (NTL/CPN) for a fantastic opportunity to join a specialist Mental Health Crisis Home Treatment Team based in Norfolk within the NHS.

Applicant must be a car driver as you will need to travel to other sites.

Temporary pay rate:
, Days: £24 per hour
, Saturdays and Nights: £30 per hour
, Sundays & Bank Holidays: £36 per hour

Hours and length of contract:
, Full time shifts, 24 hour service
o Earlies 0700-1500
o Lates 1300-2100
o Twilight 1500-2300
o Nights 2030-0730
, The manager is looking for the position to be filled ASAP with an initial 3 month contract length.

Daily Duties:
You will be working as part of the Acute Assessment and Home Treatment Team (HTT) to visit service users in their own homes with the aim to support them following discharge and reduce hospital admission.

The service operates 7 days per week for service users with severe mental illness or those in an acute mental health crisis who have been referred from Access and Assessment team, Adult Recovery Teams, acute psychiatric wards and the psychiatric liaison team.



You will provide emergency assessment of new referrals within a four hour timeframe and provide a supportive and innovative approach towards improving patient care.

Home treatment will include medication, and brief psychological interventions and referrals where necessary.
You will assess, plan, implement and evaluate packages of care based upon individual service user needs and risk and you will also undertake and maintain service users' Self Directed Support assessments.

Person specification:
The applicant must be DBS cleared and have previous experience in the field.
Knowledge of Remedy helpful.
